Oakley Forman, Sr., 77, of Petersburg, died Tuesday, March 8, 2011 at St. Elizabeth Healthcare, Edgewood. He was a retired truck driver with Wintz Trucking Company of Cincinnati.
Survivors include his wife, Peggy Ann Perkins Forman; sons, Gary Forman, Oakley Forman, Jr., and Sam Faulkner, all of Petersburg; daughter, Joy Rudloff of Mariemont, Ohio; step daughters, Sandra Jones and Rebecca Johnson, both of Charlotte, North Carolina; brothers, Buck Allen of London, Chester Allen of Somerset, and Eugene Allen of Clay County, Kentucky; sister, Lela Combs of Manchester; three grandchildren; and two great grandchildren.
He was preceded in death by his parents, Woodrow Allen and Ora Forman.
